Tests Reports of Female Partner that we require :

• Day 2 Serum Hormones - E2, FSH, LH, TSH and Prolactin

Tests Reports of Male Partner:

• Semen Analysis 
• Serum Hormones - FSH, LH, TSH and Prolactin 

Please answer the below questions, to serve you better !

• Age of both the partners? 

• How long you have been married and since when you are trying to get conceived without taking any contraceptives?

• Ultrasound Scan would give a clear picture of your Ovaries and Uterus conditoin. If you had taken one, please forward us the findings? 

• HSG ( Hysterosalpingogram ) is done to see if you have blocked fallopian tubes or if there are any abnormalities in your uterus. If you had done HSG before, what were the findings?

• Have you undergone Diagnostic laparoscopy ? If yes, what were the findings? 

• Have you done any IUI / IVF/ ICSI before? If yes, how many cycles and what were their results? 

• Have you been diagnosed positive for any Sexually Transmitted Diseases (STD's) ?    

• If you think you have other relevant information to share, please feel free to give those details.
